Required Attachments for Update/Change PAF

Based on the type of update, include the following documentation:

Rate or Salary Change
  • An SDF (Salary Data Form) is required for any regular benefits-eligible employee.
  • Increases requiring Chancellor and/or President approval must include documentation of such approval in addition to the SDF.
Appointment Extension
  • Provide an offer letter to support the extension. Note: Offer letters are not required for employees in the S2 sub-group.
FTE Reduction
  • When an FTE reduction is completed at the employee's request, the completed Voluntary FTE Reduction form must be attached. 
Faculty Leave of Absence (LOA)
  • LOA approval letter from the President’s Office. 
Faculty Salary Increase
  • Letter, email, or any form of documentation confirming EVC Office is aware and/or supportive of the increase.
